Dr. Clark, as well as his entire staff, is committed to providing the highest quality patient care at a reasonable cost. We strive to make the financial options as convenient as possible and we feel it is extremely important to communicate to our patients what they can expect. Our written Financial Agreement form explains what will be asked of you, financially, at the time of service. We would encourage you to read over this form ahead of time and would want you to feel comfortable asking any questions that you may have. Insurance

We are contracted with the following dental insurance companies:

Aetna Dental (all plans), Ameritas, Blue Cross of Oregon (Dental and Medical), Cigna Dental (PPO and HMO plans), Guardian (Dental Guard policies), Met Life, Oregon Dental Service/Delta Dental (Premier and PPO Plans).

We realize the importance of keeping your costs down and are dedicated to fully utilizing your insurance benefits for any services rendered by Dr. Clark.

Oral surgery procedures can be covered by medical or dental insurance, depending upon your insurance company and policy. Please be sure to read your insurance booklet and forms carefully, so that you will be familiar with your coverage. Please bring both your medical and dental insurance cards to your first appointment.

Dr. Clark “opted-out” of Medicare and cannot bill Medicare for any services provided. We welcome Medicare patients, however, all charges are payable by the patient.
