We ask that you call to schedule a preliminary evaluation before surgery. It will be requested of you to provide your personal information along with medical and dental insurance information. This initial appointment will consist of an assessment of your medical health history and a consultation explaining your diagnosis and treatment options. If your procedure will be relatively minor, such as an extraction under local anesthesia, Dr. Clark may determine that the surgery can be performed on the same day as the evaluation. If there is a potential for this, it will be communicated to you when you are scheduling. Otherwise, please expect this first appointment to be an evaluation only. A complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Additionally, a pre-operative consultation and physical examination is mandatory for patients who choose IV sedation for surgery. Prior to your surgical appointment, please follow the pre-operative instructions including fasting guidelines.  On the day of your surgery, you will also need an adult to drive you home and your driver will need to remain on the premises during your procedure. 

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

Please assist us by providing the following information at the time of your appointment:

  • Referral slip from your dentist or doctor
  • X-rays, if applicable
    If your dentist or physician has taken x-rays, please contact their office to request that these are forwarded to our office promptly. If there is not sufficient time to send the x-rays, please arrange to pick these up from your dentists’ office. If additional x-rays are necessary, they can be taken at our facility.
  • A list of medications that you are currently taking and a completed medical health history questionnaire (click here to download form).
    Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, cancer treatment, etc.) and make sure to list all information on your health history questionnaire.
  • Medical and dental insurance card(s)
  • Completed registration form (click here)
  • If the patient has a Power of Attorney for medical or financial purposes, it is necessary that our office has a complete copy of this prior to the initial appointment.
  • It is very helpful for patients to review the financial agreement prior to your appointment.

If you prefer that we email or mail a registration packet to you, rather than filling out the online form on our website, we will be happy to do so at the time of scheduling.

We try our best to stay on schedule to minimize your wait time, however, occasionally unforeseen occurrences can cause disruption in our schedule. For example, the need to provide emergency treatment can arise and cause delays. We appreciate your understanding and patience.
